Compliance documents required for all vendors:

  • W-9 form
  • Business license or UBI number
  • Signed vendor agreement
  • Lead certification
  • General liability insurance with Maple Leaf listed as the certificate holder


We will review all applications for consideration once required compliance documents are received.
